WebPT, Inc. (“us,” “we,” or “our”) operates the GetPT Site (“Site”).

This page informs you of our policies regarding the collection, use, and disclosure of Personal Information when you use this Site. We will not use or share your information with anyone except as described in this Privacy Policy.

We use your Personal Information for providing and improving the GetPT Site. By using this Site, you agree to the collection and use of information in accordance with this policy. Unless otherwise defined in this Privacy Policy, terms used in this Privacy Policy have the same meanings as in our Terms of Use, accessible at getpt.org

Information Collection and Use

While using the Site, we may ask you to provide us with certain personally identifiable information that can be used to contact or identify you. We collect this information for the purpose of providing services on the Site, identifying and communicating with you, responding to your requests/inquiries, and improving our services.

You can access and browse the Site at any time without providing Personal Information that can be traced back to you.

To use some of its services and access certain areas of the Site, you must provide Personal Information that can be traced back to you, such as full name, street address and zip code, telephone number, email address, gender, date of birth, and insurance coverage. We collect this Personal Information that you voluntarily provide through the use of the Physical Therapy Finder (“Physical Therapy Finder”) or when you schedule an appointment using the Physical Therapy Finder. This personally identifiable information that we collect from the Physical Therapy Finder can be traced back to you and will include or be linked or associated with health-related information, such as health conditions or treatments. The combination of this Personal Information with health-related information provided by you during use of the Physical Therapy Finder is called Protected Health Information and is subject to the special rules under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and various amending laws, including the Health Information Technology for Economic and Clinical Health (HITECH) Act. WebPT treats all personally identifiable health information received from you from the Physical Therapy Finder as Protected Health Information. Any information you include or link to anywhere else on this Site other than within the Physical Therapy Finder is not Protected Health Information and is not treated as such by WebPT under HIPAA.

WebPT may share your Personal Information: (1) with third parties who have agreements with us, including other healthcare providers; (2) if WebPT is required by law to do so; (3) in the event of a transfer of ownership of WebPT or GetPT, merger, or other similar transaction (including one in connection with any bankruptcy or similar proceedings); or (4) as otherwise set forth in this Privacy Policy. WebPT is not and cannot be responsible for the activities or privacy policies of any third party with whom your Personal Information is shared.

WebPT may disclose Personal Information to third parties in an aggregate, anonymized manner that cannot be traced back to you. WebPT may use aggregated data to improve patient access to healthcare provider services.

Use and Disclosure of Personal Health Information

WebPT’s use and disclosure of your Protected Health Information is governed by HIPAA. When you use the Physical Therapy Finder to schedule an appointment with a selected healthcare provider, all Protected Health Information that you submit is used and disclosed by WebPT as a Business Associate (as defined by HIPAA) according to the terms of the Business Associate Agreement between us and the healthcare provider. This means that WebPT may only use and disclose your Protected Health Information on behalf of, or to provide services to, the healthcare provider for the appointment scheduling services available through the Physical Therapy Finder according to the terms of the Business Associate Agreement. WebPT also may use and disclose your Protected Health Information (1) for its internal management and administration; (2) to carry out its legal responsibilities; and (3) to perform certain data aggregation services for the healthcare provider and other healthcare providers. Before disclosing your Protected Health Information for WebPT’s internal management and administration or to carry out our legal responsibilities, WebPT will obtain satisfactory assurances from the person to whom the Protected Health Information is disclosed that it will be held confidentially and used or further disclosed only as required by law or for the purpose for which it was disclosed to that person.

As a Business Associate of a particular healthcare provider, WebPT may be permitted to de-identify your Protected Health Information created or received by WebPT for the healthcare provider. The de-identified data must be created according to the criteria established by HIPAA so there is no reasonable basis to believe this information can be traced back to you. WebPT may use and disclose any de-identified information in the same manner as Personal Information under this Privacy Policy.

Log Data

We collect information that your browser sends whenever you visit our Site (“Log Data”). This Log Data may include information such as your computer’s Internet Protocol (“IP”) address, browser type, browser version, the pages of our Site that you visit, the time and date of your visit, the time spent on those pages, and other statistics.

Additionally, we may use third-party services such as Google Analytics that collect, monitor, and analyze this type of information in order to increase the Site’s functionality. These third-party service providers have their own privacy policies addressing how they use such information.

Cookies

Cookies are files with a small amount of data, which may include an anonymous unique identifier. Cookies are sent to your browser from a website and transferred to your device. We use cookies to collect information in order to improve our services for you.

WebPT uses remarketing services to advertise on third-party websites to you after you visited our Site. We, and our third party vendors, use cookies to inform, optimize, and serve ads based on your past visits to our Site.

Only persons age 18 or older have permission to access our Site. Our Site does not address anyone under the age of 13 (“Children”).

If we become aware that we have collected Personal Information from Children under age 13 without verification of parental consent, we will take steps to remove that information from our servers.
